Room Usage Request Form

By filling out and completing this form, you agree to the following User Obligations:

Receive training in use of multi-media equipment
Set up furniture to meet the needs of the event
Restore furniture to original configuration
Dispose of all trash
Vacate space no later than 5:00 p.m.

In return, P-ADL is obligated to:

Train room users in the use of room multimedia
Provide access and lock-up of room during normal business hours (7:30am-5:00pm weekdays)

Telecommunication Equipment

P-ADL has available a self-contained Polycom video conferencing unit (model VSX-7000) that can be deployed in any of our 2nd floor rooms using the UCF IP network. The unit is provided at no charge, and is furnished with a 32″ LCD monitor on a rolling media cart.

P-ADL also has available a tabletop Polycom SoundStation audio teleconferencing unit that can be provisioned in event and meeting rooms with telephone connections, specifically rooms 209, 208 and 235.
